No doctor-patient relationship through the site

Submitting a contact form, referral, or appointment request does not by itself establish a doctor-patient relationship. We establish that relationship in writing after we verify insurance, confirm we serve your area, and schedule a visit.

Forms and submissions

When you use the Contact, Refer-a-Patient, or similar forms, you confirm that the information you provide is accurate and that you have the right to share it. Do not submit detailed protected health information (PHI) through web forms. If you need to discuss something clinical, please call us at 562-686-2015 or wait until we follow up by phone.

Coverage and eligibility

We accept Medicare and select insurance plans. Coverage for any specific visit or service depends on your eligibility, medical necessity, and your plan's rules. We verify your benefits before scheduling so there are no surprises. Nothing on this site is a guarantee of coverage or a quote of cost.

Intellectual property

The Mobile Health Providers name, logo, copy, and original photography on this site are owned by Mobile Health Providers or licensed to us.
