This policy describes how Mobile Health Providers ("we," "us," "our") handles information collected through this website. It does not cover the protected health information (PHI) you share with us in the course of treatment. PHI is governed by federal HIPAA rules and our separate Notice of Privacy Practices, which we provide at intake and will send to you on request.

- Information you give us. When you use the Contact or Refer-a-Patient forms, you choose what to share, typically a name, email, phone number, ZIP code, and a short note about the situation. Do not include sensitive medical details in these forms; we will follow up by phone for anything clinical.
- Information collected automatically. Like most websites, our host records standard technical information such as your IP address, browser type, the pages you view, and the date and time of your visit. This is used for security, troubleshooting, and aggregate site analytics.
- Cookies. The site uses a small number of functional cookies (for example, to remember if you have dismissed a banner). We do not currently use advertising or cross-site tracking cookies.

Children
This website is not directed to children under 13, and we do not knowingly collect personal information from children through it. If you believe a child has submitted information, contact us and we will delete it.
