House Calls

Primary care for patients who cannot easily leave home.

Routine visits, chronic disease management, medication reviews, and annual wellness visits. Built for homebound and mobility-limited patients who need a real primary care physician relationship, just not the trip to the office.

We manage diabetes, hypertension, heart failure, COPD, and other chronic conditions. We review your medications carefully and look for interactions. We help with everything a traditional primary care office would do, just from your living room or bedside.

Who this is for

Adults who are homebound or have a hard time getting to medical appointments due to mobility, illness, or caregiving needs. Most patients are Medicare beneficiaries 65 and older.

House Calls

Wound care, tailored to the wound.

Every wound is different. We start with a thorough evaluation, then build a treatment plan tailored to your specific wound, your health conditions, and your goals. Some wounds respond to simple therapies. Others need more advanced options. We bring the full range to your bedside.

We see diabetic foot ulcers, pressure injuries, venous leg ulcers, surgical wounds that will not heal, and complex chronic wounds. When your wound needs more, we have the tools: therapeutic ultrasound to promote healing, diagnostic imaging to assess tissue health, biologic wound coverings, and compression therapy.

Who this is for

Patients with chronic or non-healing wounds who would otherwise need transport to a wound care center. Homebound patients. Residents of skilled nursing, assisted living, board and care, and independent living facilities.

Between visits

Care between visits, not just during them.

Your doctor's visit is just one piece. We stay connected with you between visits through regular check-ins, remote monitoring of vitals, medication reviews, and mental health support. The goal is catching problems early so they stay small.

That means a nurse calling to check on your blood pressure readings. A care coordinator following up on the medication change your doctor made last week. A check-in about how you have been feeling, not just physically. This is how we prevent the small problems from becoming hospital visits.

Who this is for

Patients with two or more chronic conditions, or anyone who would benefit from closer monitoring between in-person visits. For eligible patients, Medicare typically covers this service with minimal or no copay. We confirm coverage and any patient responsibility before enrollment.

Video visits

Telemedicine for follow-ups and quick check-ins.

Some visits don't need a knock at the door. For follow-up appointments, medication changes, lab result reviews, and triage of new concerns, a video visit works well, and it means faster access to your care team.

We use telemedicine to complement, not replace, our in-person visits. The combination keeps care continuous without making you wait for the next house call.

Coverage note

Telemedicine is subject to Medicare coverage rules. Current Medicare telehealth flexibilities for home-based care are extended through December 31, 2027. We verify your specific coverage before any visit.

Connected care

One team. Everything you need without leaving home.

When you need services beyond what we provide, we connect you with partners who come to you, too. Hospice, home health nursing, mobile radiology for X-rays and imaging at home, and laboratory services for blood draws at home.

One team, one goal: keeping you healthy at home. For referring providers, this is the assurance that your patient will not fall through the cracks after discharge.

Who this is for

Any patient whose care plan involves multiple home-based services. We coordinate the clinical handoffs so you do not have to manage them.

In the facility

Facility visits: SNF, assisted living, board and care, independent living.

We provide primary care and wound care for residents of skilled nursing facilities, assisted living, board and care homes, and independent living communities. We coordinate directly with your nursing staff and attending physicians, and we document in the format your facility needs.

For administrators, executive directors, and DONs: we work hard to be the kind of partner that makes your day easier, not harder. Show up on time. Communicate clearly. Document promptly.

Administrators, EDs, DONs, and care coordinators at SNF, ALF, RCFE/board and care, and independent living facilities looking for a reliable on-site primary care and wound care partner. Covered by Medicare.
